This is Homer Winslow's wood engraving of The Surgeon At Work At The Rear During An Engagement that was published in 1862. Surgeons in those days were not at the top of their profession and indeed were looked down upon by regular doctors who did not bother themselves with the mechanical tasks of cutting and sewing. Most doctors had only a rudimentary education (you didn't have to have a college degree to be admitted to medical school) and many surgeons were recruited from among butchers and barbers. Below is a picture of the left bottom corner showing Homer's signature.
